Electronic Signature, Applicant Certification, At-Will Employment, and Background Check Authorization

By checking the acknowledgment box and electronically signing this Employment Application, I understand and agree to the following:

Electronic Signature Consent

I consent to the use of electronic records and electronic signatures in connection with this Employment Application and any related employment documents. I understand that my electronic signature has the same legal effect and enforceability as my handwritten signature under applicable federal and Indiana law. By electronically signing this application, I certify that I am the individual identified in this application and that I am authorized to provide this electronic signature.

Applicant Certification

I certify that all information provided in this Employment Application, including any attachments, resumes, supporting documents, and statements made during the hiring process, is true, complete, and accurate to the best of my knowledge.

I understand that any omission, falsification, misrepresentation, misleading statement, or material inaccuracy in any information provided may result in disqualification from consideration for employment or, if employed, disciplinary action up to and including immediate termination of employment.

Authorization to Verify Information

I authorize the Company to investigate and verify all statements contained in this application and any supporting materials, including but not limited to employment history, educational background, professional licenses, certifications, references, and other information relevant to my qualifications for employment, to the extent permitted by applicable law.

Background Check Authorization

I understand that, as a condition of employment or continued employment, the Company may conduct a background investigation consistent with applicable federal, state, and local laws. I authorize the Company and its designated agents to obtain information concerning my background, including criminal history, employment history, education verification, and other lawful records relevant to employment decisions. I understand that any required disclosures and authorizations under the Fair Credit Reporting Act (FCRA) and other applicable laws will be provided separately when required.

At-Will Employment Acknowledgment

I understand and acknowledge that, if hired, my employment with the Company will be on an at-will basis unless otherwise provided by a written agreement signed by an authorized representative of the Company. This means that either I or the Company may terminate the employment relationship at any time, with or without notice, and with or without cause, subject to applicable law.

I further understand that no oral statements, policies, practices, or representations made by any employee or representative of the Company shall alter the at-will nature of employment unless expressly set forth in a written agreement signed by an authorized Company representative.

Acknowledgment

By electronically signing the application, I acknowledge that I have read, understood, and agree to the terms set forth above. I further acknowledge that my electronic signature constitutes my legal signature and that I intend to be bound by the statements and authorizations contained herein.
